Horny goat weed has interesting preclinical mechanisms but limited robust human outcome data.
Horny goat weed commonly refers to Epimedium species. The compound icariin and related flavonoids have attracted attention because laboratory studies have explored phosphodiesterase-5-related activity and other vascular mechanisms.
That preclinical work is scientifically interesting, particularly because PDE5 is also a pharmacological target in established erectile-dysfunction medicines. But demonstrating enzyme activity in a laboratory setting is not the same as showing that a commercial horny-goat-weed dose produces a clinically meaningful response in men.
